To refer a patient, simply complete and submit a referral form.
After reviewing, we will contact the patient to introduce ourselves and book them in. You will be kept fully updated on progress throughout.

Complete referral form with patient details.

Appointment
We confirm patient details and schedule a consultation with them. We advise the dental practice of the appointment.

Treatment plan
We outline the treatment plan, send it to the patient and their dental practice.

Treatment report
Once the patient is treated, we refer them back to their dental practice together with the treatment report.
